In signing and returning this form to Virtuoso Voice Academy you are agreeing to the following terms and conditions.

The Student/The Child, may be photographed, videoed, or recorded, for use in all social mediums permitted under your consent, plus:

Social platforms such as Virtuoso Voice Academy’s Facebook & Instagram pages
Virtuoso Voice Academy website - (this is an open domain / www.virtuosovoiceacademy.com)

OTHER Published material may include (not limited to)

Public Newsletters within the Sutherland Shire / St George areas
Live performances / annual concerts
Events / Concerts / LIVE Performances and Recordings
Public publications and promotional material online

Public media events that may include but not limited to:

Participation in musical or dramatic performances within the community and/ or participation in community events, musicals and events such as fairs, fates, school fates in Sutherland shire / St George area.

I authorise and give permission for Virtuoso Voice Academy to take and use any photographs, videos or sound recordings of the student / my child and any other reproductions or adaptations of the student’s/my likeness or the student’s/my work (‘the material’), or the student’s/my name, either in full or part, in any Virtuoso Voice Academy authorised or associated publication, production and presentation, which may include publications on public websites operated by The Virtuoso Voice Academy, social media sites such as the studios Facebook page and Instagram Profile. Parents should be aware that when information is published on public websites and social media channels, it can be discoverable online for several years, if not permanently. Search engines may also cache or retain copies of published information. Published information can also be linked to by third parties.
